Office Policies

Cancellations / No-Shows

Please remember that we reserve the appointment time especially for you. If you are unable to keep the appointment, please kindly provide us with 24 hours of notice so that we can make this time available to another patient. If you no-show your appointment, you may be charged $40.

Co-Pays

All co-pays are due at the time of service. We accept cash, personal checks, Visa, MasterCard and Discover.

Prescription Policies

We encourage patients to bring all maintenance prescriptions with you to any office visit so that we can review and refill your medications during your regular follow up visits. Refills of maintenance medications will be handled during regular office hours only. Requesting refills during office hours allows us to review your medical record and ensure that a refill is the best course of treatment for you. We will refill your medications or call you if there are any questions within 24 hours. Although we pledge to address your prescription refill needs promptly, we encourage you to request refills a few days in advance of when you will need them and to anticipate your need for refills before weekends or holidays.

Emergencies

Our providers are available 24 hours a day, 7 days a week to provide advice about your medical problems. Please respect their time by only calling after office hours with questions regarding acute problems.
You may reach our physicians after hours by calling the main office number 847-742-3120. The phone system will instruct you to leave a message. Please speak clearly and be sure to leave your name and phone number. A physician will return your call within 30 minutes. If you have not heard back from a doctor in this time frame, please call again noting that you have not received a response so that we can be sure that we did not miss your call.
